Job summary

Continuum Therapy Partners is seeking a PRN Physical Therapist to join the Mineral Point Health Services team in Mineral Point, WI. The role offers daytime PRN scheduling with a focus on individualized treatment and collaborative care.

Requirements include a Doctor of Physical Therapy, an active state license (NPTE), and a preference for 1 year of PT experience. You will evaluate patients, develop plans, document progress, and educate families on home exercise programs.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:
