Job summary

Continuum Therapy Partners is seeking a Physical Therapist for PRN work, offering flexibility and a supportive team led by experienced therapists. You’ll help deliver high-quality care while maintaining your clinical voice within a collaborative environment.

Responsibilities include evaluating patients, developing treatment plans, documenting progress, and educating families. Locations include Wisconsin Rapids Health Services and River Run Assisted Living in Wisconsin Rapids, WI.

Qualifications

• DPT degree is required.
• Current state PT license and NPTE completion required.
• At least 1 year of PT experience preferred.

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required


• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)


• 1 year of previous experience as a Physical Therapist preferred


• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines


• Documents patient progress and adjusts plans of care based on clinical outcomes


• Educates patients and families on home exercise programs and injury prevention
