Job summary

Continuum Therapy Partners is seeking a Physical Therapist for PRN daytime coverage at Green Bay Health Services in Green Bay, WI. You will join a clinical team that values your voice and provides mentorship while supporting flexible scheduling.

The role requires a Doctor of Physical Therapy, an active Wisconsin PT license, and NPTE eligibility, with 1 year of prior PT experience preferred. This PRN position offers daytime shifts in a supportive setting.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
