Job summary

Continuum Therapy Partners is seeking a Physical Therapist in Sheboygan, WI for a PRN daytime role. You will join a team of clinicians dedicated to high-quality care, with the flexibility to schedule around your life while contributing to patient outcomes.

The position requires a Doctor of Physical Therapy, a current state license, and ideally 1 year of PT experience. You will evaluate patients, develop tailored treatment plans, document progress, and educate patients and families on home

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
