Job summary

Continuum Therapy Partners is seeking a Physical Therapist for our PRN team in South Milwaukee, WI. You’ll choose your daytime schedule while remaining part of a care-focused therapy group.

The ideal candidate holds a Doctor of Physical Therapy (DPT) degree, an active Wisconsin PT license, and NPTE; a minimum of 1 year of experience is preferred. This role provides daytime PRN shifts at Willowcrest Health Services.

Qualifications

• DPT degree is required.
• Active Wisconsin Physical Therapy license is required; NPTE must be passed.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans.
• Documents patient progress and adjusts plans of care.
• Educates patients and families on home programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
