Job summary

Continuum Therapy Partners is seeking a PRN Physical Therapist to join the Hammond Health Services team in Hammond, WI. You will evaluate patients, develop and implement individualized treatment plans, and work with a multidisciplinary team to ensure quality care.

The role emphasizes flexible daytime PRN scheduling, licensure in the state, and adherence to NPTE requirements, with a preference for candidates with 1 year of PT experience. This position is on-site in Hammond, Wisconsin.

Qualifications

• Doctor of Physical Therapy (DPT) required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:
