Job summary

Continuum Therapy Partners, LLC seeks a Physical Therapist to join their team on a PRN basis in Cedarburg, WI. The role emphasizes flexible daytime hours while maintaining high-quality patient care and collaboration with therapists who understand clinical realities.

Candidates must hold a DPT, a current Wisconsin license (NPTE passing status anticipated), and prefer at least one year of prior PT experience. The position is based at Cedarburg Health Services with on-site care delivery.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities
