Job summary

Continuum Therapy Partners is seeking a licensed Physical Therapist to join our team at Regirer Park Health & Rehab Center in Richmond, VA. The role emphasizes patient-centered care, collaboration with a multidisciplinary team, and delivering individualized treatment plans.

The ideal candidate holds a Doctor of Physical Therapy, a current Virginia license, and NPTE eligibility. Prior clinical experience is preferred, with a focus on education and injury prevention for patients and families.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
