Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our clinician-led team serving Buena Vista, VA. The role emphasizes evaluating and treating patients with individualized plans of care and collaboration with other disciplines.

The ideal candidate holds a Doctor of Physical Therapy (DPT), an active state license, and NPTE. Prior PT experience is preferred, with a commitment to patient education and outcome-driven practice.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
