Job summary

Continuum Therapy Partners, LLC in Virginia seeks a licensed Physical Therapist to join our clinician-led team at Galax Health and Rehab.

The role involves evaluating patients, developing and implementing individualized treatment plans, and coordinating with other disciplines, with a daytime PRN schedule. A DPT and NPTE license are required, with 1 year of experience preferred.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; NPTE required.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes; Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities
