Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist for PRN coverage at Whitewater Health Services in St. Charles, MN.

The successful candidate will hold a Doctor of Physical Therapy (DPT) and a current state license, pass the NPTE, and have prior PT experience to evaluate and treat patients in coordination with the care team. Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
