Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our team at Monroe, WI. The role emphasizes partnership with patients and families to elevate quality of care in a multi-location network.

Requirements include a Doctor of Physical Therapy, an active state license (NPTE), and preferred 1 year of PT experience. The schedule is part-time daytime at Monroe Manor Nursing & Rehab Center.

Qualifications

• Current, unencumbered state Physical Therapy License; NPTE required.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
