Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our growing network. This role focuses on evaluating and implementing individualized treatment plans, coordinating with other disciplines to optimize patient outcomes.

The ideal candidate holds a Doctor of Physical Therapy, a current state license, and has preferred 1 year of experience. PRN daytime shifts at Wausau, WI locations support flexible scheduling within our extensive care network.

Qualifications

• DPT degree is required.
• Current, unencumbered state PT license; must pass NPTE.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
