Job summary

Continuum Therapy Partners seeks a Physical Therapist to join our PRN team at Brynwood Health & Rehabilitation Center in Monticello, FL. The role offers flexible daytime PRN hours, allowing you to tailor a schedule around your life while delivering quality care.

Candidates must hold a DPT, current FL license, and NPTE, with a preference for 1 year of PT experience. You’ll evaluate and implement individualized treatment plans, document progress, and educate patients and families on home exercises.

Qualifications

• DPT required.
• Active Florida PT license and NPTE.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Education

Doctor of Physical Therapy (DPT) degree

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
