Job summary

Continuum Therapy Partners is seeking a PRN Physical Therapist to join a team dedicated to quality care. You decide when you work, while remaining part of a collaborative group led by experienced therapists. Start date is 1/1/27, with flexible daytime PRN shifts in St.

Augustine, FL. Requirements include a Doctor of Physical Therapy (DPT), an active Florida PT license (NPTE must be passed), and 1 year of prior PT experience preferred.

Qualifications

• DPT degree required.
• Current, unencumbered state PT license; must pass NPTE.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines

• Documents patient progress and adjusts plans of care based on clinical outcomes

• Educates patients and families on home exercise programs and injury prevention
