Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist in St. Augustine, FL for a PRN daytime schedule. The role supports a collaborative team led by licensed therapists who value flexible work arrangements and quality patient care.

The successful candidate will hold a Doctor of Physical Therapy (DPT), an active state license, and NPTE eligibility. Prior PT experience is preferred to help evaluate and implement individualized care plans and educate patients on home exercises.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
