Job summary

Continuum Therapy Partners seeks a Physical Therapist to join as a PRN provider at Lake City, FL. You will work with a team of clinicians to deliver high-quality care, choosing your own daytime schedule while contributing meaningfully to patient outcomes.

Requirements include a Doctor of Physical Therapy (DPT), current FL license (NPTE), and a proactive approach to home exercise programs and injury prevention. PRN hours are daytime.

Qualifications

• DPT degree required.
• NPTE license is required to practice.
• 1 year of PT experience preferred, not mandatory.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines

• Documents patient progress and adjusts plans of care based on clinical outcomes

• Educates patients and families on home exercise programs and injury prevention
