Job summary

Continuum Therapy Partners in Starke, FL seeks a Physical Therapist for our PRN program. You choose when you work, yet remain part of a supportive team focused on quality patient care.

Requirements include a Doctor of Physical Therapy, an active state license and NPTE passage; 1 year of PT experience is preferred. You will evaluate and implement individualized plans and educate patients on home exercises.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; NPTE passed.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
