Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist for a PRN, on-site role in Grundy, VA. The candidate will evaluate and treat patients, develop and adjust plans of care, and educate families on home exercise programs and injury prevention.

Requirements include a Doctor of Physical Therapy (DPT), an active PT license, NPTE eligibility, and prior clinical experience. This position offers flexible PRN scheduling and proximity to Heritage Hall Grundy.

Qualifications

• DPT degree required.
• Current, unencumbered state PT license; NPTE must be passed.
• 1 year of experience preferred.
• Evaluates, develops, and implements individualized treatment plans.
• Documents patient progress and adjusts plans of care.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
