Job summary

Continuum Therapy Partners seeks a Physical Therapist for PRN daytime coverage at Evergreen Nursing & Rehab in Effingham, IL. A DPT and active state license are required; NPTE must be passed. One year of PT experience is preferred, with mentorship and team-led support.

You will evaluate and implement individualized treatment plans, document patient progress, and educate patients and families on home exercise programs and injury prevention.

Qualifications

• Must hold a Doctor of Physical Therapy (DPT) degree.
• Active state Physical Therapy License; NPTE must be passed.
• 1 year of PT experience preferred.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
