Job summary

Continuum Therapy Partners, LLC in Hammond, Wisconsin seeks a Physical Therapist for a PRN role. You choose when you work while remaining part of a team led by experienced clinicians focused on high-quality care.

Requirements include a Doctor of Physical Therapy (DPT), an active Wisconsin PT license, NPTE, and preferably 1 year of PT experience. You will evaluate and implement individualized treatment plans and educate patients on home exercise programs to prevent injuries.

Qualifications

• DPT degree required.
• Current, unencumbered state PT license; must pass the National Physical Therapy Examination (NPTE).
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
