Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist for PRN daytime shifts at Evergreen Nursing & Rehab in Effingham, IL. You’ll join a team focused on high-quality care and flexible scheduling that respects your clinical voice.

Requirements include a Doctor of Physical Therapy, active IL license with NPTE, and 1 year of PT experience preferred. You will evaluate and tailor treatment plans, document progress, and educate patients and families on home exercise programs.

• Current, unencumbered IL PT license; NPTE must be passed.
• 1 year PT experience preferred.
• Evaluates, develops and implements individualized treatment plans with other disciplines.
• Documents progress and adjusts plans based on outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
