Continuum Therapy Partners is seeking a Physical Therapist to join our Le Sueur, MN team on a part-time daytime schedule. A DPT and current MN PT license are required, with NPTE passage and 1 year of experience preferred. You will evaluate patients, develop and implement care plans, document progress, and educate families on home exercise programs.

The role offers a supportive environment and opportunities to collaborate across disciplines while delivering high-quality patient care.

• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
