Continuum Therapy Partners, LLC is seeking a Physical Therapist in Willmar, Minnesota. The role requires a Doctor of Physical Therapy and an active MN license or eligibility, with NPTE completed.

A minimum of one year of PT experience is preferred as you assess patients, design individualized treatment plans, and work with a multidisciplinary team to achieve optimal outcomes. Core duties include implementing plans of care, documenting progress, and educating patients and families on home

• Evaluates, develops, and implements individualized treatment plans.
• Educates patients and families on home exercise programs and injury prevention.

Responsibilities

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
