Continuum Therapy Partners is seeking a Physical Therapist for La Crescent, MN. The role focuses on evaluating and implementing individualized treatment plans, coordinating with other disciplines to improve patient outcomes, and educating patients and families on home exercise programs and injury prevention.

Qualifications include a Doctor of Physical Therapy (DPT) degree, a current state PT license with NPTE, and ideally 1 year of PT experience.

• Doctor of Physical Therapy (DPT) degree required
