Continuum Therapy Partners seeks a Physical Therapist in Willmar, MN to join a clinician-led network focused on patient-centered care. Therapists’ voices shape organizational decisions and patient outcomes.

Requirements include a DPT, MN PT license with NPTE, and ideally 1 year of PT experience. You will evaluate and implement individualized treatment plans, document progress, and educate patients and families on home programs.

Qualifications

• DPT degree required.
• Active Minnesota PT license; must pass NPTE.
• 1 year of PT experience preferred.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
