Job summary

Flagstar Rehab is actively seeking a Physical Therapist to join our rehab department in Skilled Nursing Facilities across NYC and surrounding areas. You will perform patient evaluations, develop individualized treatment plans, and collaborate with a multidisciplinary team to improve function and cognition in daily life.

The role offers options for Direct Employment or Independent Contracting, with competitive hourly pay and comprehensive support. Licensing and state requirements must be current.

Qualifications

• Licensed NYS Physical Therapist in good standing.
• CPR certification and EMR proficiency.
• Meets State and Federal requirements for employment in a nursing facility.
• Malpractice/liability insurance (if applicable).

Responsibilities

• Perform evaluations and create individualized treatment plans.
• Supervise PTAs daily in SNF settings.
• Provide therapy services for patients with TBI, CVA, Stroke, Parkinson's, MS, cancer, ortho surgeries.
• Monitor and document patient progress using EMR.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
