Flagstar Rehab is seeking Physical Therapists for Skilled Nursing Facilities in NYC and surrounding areas. Join a diverse, multi-disciplinary team to evaluate, treat, and educate patients to improve mobility and cognition.

Qualifications

• NYS Physical Therapist license and registration.
• CPR certification.
• EMR competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to SNF patients with conditions such as TBI, CVA, stroke, Parkinson's, MS, cancer, and orthopedic concerns.
• Monitor and document patient progress using EMR software.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
