Flagstar Rehab is seeking Physical Therapists to join our skilled nursing facility collaborations in NYC and surrounding areas. You will evaluate patients, develop individualized treatment plans, and supervise PTAs while delivering high-quality therapy to SNF residents.

Applicants must hold an active NYS PT license, CPR certification, and be proficient with EMR software. We offer flexible schedules, competitive hourly rates, and weekly pay with direct deposit.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.

Physical Therapist Requirements:

• Current NYS PT License and Registration
