Job summary

Flagstar Rehab is seeking Physical Therapists (PTs) for Skilled Nursing Facilities in New Jersey. The role focuses on evaluating residents, developing individualized treatment plans, and supervising PTAs. Emphasis on EMR documentation and adherence to state/federal requirements.

Positions include Full-Time, Part-Time, and Per Diem with a focus on timely, high-quality patient care. Independent contractors are not eligible for all benefits; strong support and onboarding provided.

Qualifications

• Current NJ PT license and registration required.
• BLS certification required.
• EMR software competence necessary.
• Compliance with state and federal employment for US nursing facilities.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs under supervision.
• Deliver quality therapy services to SNF residents with various conditions and post-surgical needs.
• Document patient progress using EMR software and maintain accurate records.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Requirements and Responsibilities:

• Current NJS PT License and Registration


• Valid BLS Certification


• EMR Competence


• Meets all State and Federal requirements for US employment in a Nursing Facility


• Malpractice/Liability Insurance (if applicable)


• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.


• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.


• Monitor and document patient progress with the use of innovative EMR software.
