Job summary

Flagstar Rehab is seeking Physical Therapists (PTs) for Skilled Nursing Facilities in New Jersey. The role offers flexible schedules including Full-Time, Part-Time and Per Diem with weekly pay and comprehensive benefits.

Must hold NJ PT license and BLS, with EMR competence and ability to supervise PTAs. Responsibilities include evaluations, treatment planning, and delivering quality therapy to SNF residents with diverse conditions.

Qualifications

• Current NJS PT License and Registration.
• Valid BLS Certification.
• EMR Competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.
• Provide quality therapy services to individuals in a SNF setting with conditions such as TBI, CVA, Stroke, Parkinson’s, MS, Cancer, orthopedic surgery.
• Monitor and document patient progress with EMR software.

Hourly Pay Rate: $43-$60

Pay rate commensurate with experience

Physical Therapist Requirements and Responsibilities:

• Current NJS PT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
