Job summary

Flagstar Rehab is seeking a Physical Therapist (PT) for skilled nursing facilities in NYC and surrounding areas. You will evaluate patients, design and supervise PTAs, and provide education to patients and families to improve function and cognition. The role supports diverse, multidisciplinary teams in state-of-the-art SNFs.

Compensation includes hourly pay with W-2 or 1099 options, plus benefits and weekly pay. Flexible schedules include full-time, part-time, per diem, days and evenings.

Qualifications

• Current NYS PT License and Registration.
• CPR Certification.
• EMR Competence.

Responsibilities

• Provide efficient evaluations and develop individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Deliver quality therapy services to SNF patients with conditions like TBI, CVA, Stroke, Parkinson’s, MS, cancer, orthopedic surgery.
• Monitor and document patient progress using EMR software.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.


• Supervise PTAs daily.


• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.


• Monitor and document patient progress with the use of innovative EMR software.
