Job summary

Flagstar Rehab is seeking Licensed Physical Therapists for Skilled Nursing Facilities in New York. Therapists control schedules and may choose assignments that fit their lifestyle, with independent contractor options and a W-2 or 1099 earning model. Onboarding is conducted remotely for a smooth transition.

Pay rates are $46–$48 hourly for W-2 positions and $57–$60 hourly for 1099 roles. Full-time, part-time, and per diem shifts are available, with benefits and family-friendly scheduling options

Qualifications

• Active NYS PT license and registration.
• Current BLS certification required.
• Experience delivering evaluations and individualized treatment plans.
• Malpractice/liability insurance where applicable.
• Eligible to work in US nursing facilities.

Responsibilities

• Provide efficient evaluations and develop treatment plans for PTAs to follow.
• Deliver quality therapy services to SNF residents with geriatric conditions.
• Monitor progress and document outcomes using EMR software.

Physical Therapist Requirements and Responsibilities:

• Current NYS PT License and Registration

• Valid BLS Certification

• EMR Competence

• Meets all State and Federal requirements for US employment in a Nursing Facility

• Malpractice/Liability Insurance (if applicable)

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.

• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

• Monitor and document patient progress with the use of innovative EMR software.
