Flagstar Rehab is seeking Physical Therapists to work in Skilled Nursing Facilities across NYC and Long Island. The role involves evaluating patients, planning individualized treatments, and supervising PTAs in a collaborative, multi-disciplinary team.

Employment options include direct agency roles or Independent Contractor arrangements. Compensation includes a pay rate of $46-$48 per hour (W-2) or $57-$60 per hour (1099).

Qualifications

• Current NYS PT License and Registration is required.
• CPR Certification is required.
• EMR software proficiency is expected.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Deliver therapy services to SNF patients with conditions such as TBI, CVA, or Parkinson’s.
• Monitor and document patient progress using EMR software.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
