Continuum Therapy Partners, LLC is seeking a full-time Physical Therapist at our Thief River Falls, MN location. The role focuses on evaluating and treating patients with individualized plans and close coordination with a multidisciplinary team. Strong documentation and patient education are essential.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
