Job summary

Continuum Therapy Partners is seeking a Physical Therapist at Heritage Manor- Chisholm, MN (split with Hibbing, MN). This full-time daytime role offers a competitive benefits package and an $8,000 sign-on bonus.

You will evaluate and implement individualized treatment plans, educate patients, and collaborate with other disciplines to optimize outcomes. A DPT and NPTE eligibility are required, with 1 year of experience preferred.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; NPTE required.
• 1 year of previous experience as a Physical Therapist preferred.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred
