Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our patient-focused team at Heritage Manor in Chisholm, MN. This full-time, daytime role offers comprehensive benefits and a sign-on bonus for new clinicians.

You will evaluate, develop, and implement individualized treatment plans, document patient progress, and educate patients and families on home exercise programs and injury prevention. A DPT and MN licensure are required, with NPTE completion expected.

Qualifications

• DPT is required.
• Active state license and NPTE completion required.
• Experience as PT preferred.

Responsibilities

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
