Job summary

Continuum Therapy Partners in Minnesota is seeking a Physical Therapist to join a growing network of 275 locations, dedicated to elevating quality of life for patients in skilled care. This PRN role is based at Whitewater Health Services in St.

Charles, MN, and offers meaningful clinical collaboration across disciplines. The successful candidate will hold a Doctor of Physical Therapy, a current MN license (NPTE required), and preferably one year of PT experience.

Qualifications

• DPT degree required.
• Current, unencumbered state PT license; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
