Job summary

Continuum Therapy Partners in Wisconsin is seeking a part-time Physical Therapist to evaluate and treat patients, develop personalized plans, and coordinate care with other disciplines. You will document progress and adjust plans based on outcomes.

The role requires a DPT, an active state license (WI) and NPTE, with 1 year of experience preferred. Generous PTO and 401k with company match accompany the position.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
