Job summary

Continuum Therapy Partners in West Carrollton, OH is seeking a PRN Physical Therapist to join Elmwood Health & Rehab Center. You will provide high-quality care with daytime hours, about 15-20 hours per week, and collaborate with therapists to tailor treatment plans.

Candidates must hold a Doctor of Physical Therapy, an active state license, and NPTE; one year of experience preferred. You will educate patients and families and document progress to inform ongoing plans.

Qualifications

• DPT degree is required.
• Current, unencumbered state license; NPTE required.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements treatment plans.
• Documents patient progress and adjusts plans of care.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
