Job summary

Telos Health Systems is seeking a Psychotherapist (LCSW, LCMHC, LMFT, Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services in Skilled Nursing Facilities and Assisted Living Facilities. The role offers flexibility, clinical autonomy, and strong administrative and leadership support.

This position includes travel across NC locations with a sign-on bonus and comprehensive benefits for full-time staff.

Qualifications

• Licensed mental health professionals with relevant licensure in practice state.
• Master’s or Doctoral degree in mental health field.
• Active, unrestricted license in the state of practice.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a multidisciplinary team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff and healthcare professionals.
• Adhere to federal, state, and organizational standards.
