Telos Health Systems is seeking a Psychotherapist (LPCC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to perform mental health evaluations and provide psychotherapy in Skilled Nursing Facilities and Assisted Living Facilities. The role offers flexible scheduling, clinical autonomy, and strong administrative support.

Responsibilities include assessments, individual therapy, treatment planning, and coordination with a multidisciplinary team, ensuring accurate documentation and regulatory

Qualifications

• Active North Carolina license in LPCC, LMFT, or Psychologist (PsyD/Ph.D.)
• Experience with geriatric populations in SNF/AL settings
• Strong ability to work independently within a clinical team

This role offers flexibility, clinical autonomy, and strong administrative and leadership support.

Key Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and other healthcare professionals.
• Adhere to all federal, state, and organizational standards, policies, and regulations.

Qualifications

Required

• Active North Carolina license as one of the following:
  • Licensed Professional Clinical Counselor (LPCC)
  • Licensed Marriage and Family Therapist (LMFT)
  • Licensed Psychologist -PsyD/Ph.D.
• Master’s degree (LPCC, LMFT) or Doctoral degree (Psychologist) in a relevant mental health field.
• Ability to work independently while collaborating effectively within a clinical team.
• Strong communication and clinical documentation skills.

• Experience working with geriatric populations.
• Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.
• Pay-per-encounter, fee-for-service compensation.
• Flexible scheduling with a focus on work-life balance.
• No billing or authorization responsibilities (handled by our billing team).
• Ongoing support from Directors of Mental Health and administrative staff.
