Job summary

ViaQuest Psychiatric & Behavioral Solutions is seeking compassionate RNs or LPNs to join our multidisciplinary team delivering patient-centered care in long-term care facilities.

You will perform weekly to monthly assessments, educate clients and caregivers on medications, coordinate with physicians, pharmacies, and treatment teams to ensure quality outcomes.

Travel across the service area is required, with opportunities to mentor peers and contribute to care management across locations.

Qualifications

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Preferably has experience working in LTC facilities as well as Behavioral Health.
• CPR or Course can be provided during training.

Responsibilities

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in person nursing assessments as well as medication and chart review in long term care facilities (LTC).
• Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
• Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
• Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May need to cover more than one facility depending on company needs
