Job summary

ViaQuest Psychiatric & Behavioral Solutions in Ohio is seeking a dedicated Psychiatric RN/LPN to join our LTC team. You will conduct in-person nursing assessments, review medications, and educate caregivers and clients on prescribed psychiatric and physical health treatments in long-term care facilities.

The role emphasizes care coordination, weekly to monthly health management, and support across multiple locations.

Qualifications

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Preferably LTC facilities experience with Behavioral Health.
• CPR certification or training provided during orientation.

Responsibilities

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in-person nursing assessments and medication reviews in LTC facilities.
• Educate caregivers and clients on prescribed psychiatric and physical health medications and monitor for side effects.
• Care management duties including coordination of community services and collaboration with treatment team members.
• Assist clients in building coping skills and achieving annual goals related to their psychiatric diagnosis.
• May cover more than one facility and train/mentor peers.

Responsibilities may include:

• Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
• Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
• Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May need to cover more than one facility depending on company needs

Requirements for this position include:

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Preferably has experience working in LTC facilities as well as Behavioral Health.
• CPR or Course can be provided during training
