Qualifications

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Prefer LTC facility and Behavioral Health experience.
• CPR training may be provided during training.

Responsibilities

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in-person nursing assessments and medication/chart reviews in LTC facilities.
• Educate caregivers and clients on prescribed psychiatric and physical health medications and monitor for side effects.
• Care management duties including coordination with treatment team members, physicians, pharmacies and employment programs.
• Assist clients in building coping skills and creating annual goals related to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May need to cover more than one facility depending on company needs.

At ViaQuest Psychiatric & Behavioral Solutions we offer unique and individualized care to clients in the long term care settings, with multiple diagnoses, including dementia, anxiety, depression, psychosis and more. Our clients are supported by a team of Registered Nurses (RNs),Licensed Practical Nurses (LPNs), certified nurse practitioners, psychiatrists, mental health counselors, social workers, and case managers to ensure the best outcomes for those we serve.

This position is based in a long-term care facility focused on providing mental health services to residents. The role requires flexibility, empathy, and a commitment to improving the quality of life for individuals with psychiatric needs.

Responsibilities may include:

Requirements for this position include:

• Graduate of an accredited nursing program.
• LPN or RN license.

• Preferably has experience working in LTC facilities as well as Behavioral Health.
• CPR or Course can be provided during training
