Job summary

ViaQuest Psychiatric & Behavioral Solutions in Ohio seeks a dedicated RN/LPN to provide comprehensive nursing care in long-term care settings. You will assess, monitor medications, educate caregivers, and coordinate with a multidisciplinary team, ensuring the best outcomes for clients.

This full-time role may require travel across the service area. You will join a collaborative team with opportunities for mentorship and ongoing training, including CPR when needed.

Qualifications

• Graduate of an accredited nursing program.
• LPN or RN license.
• Willingness to travel throughout assigned service area.
• Preferably LTC or Behavioral Health experience.
• CPR certification or training provided during onboarding.

Responsibilities

• Address mental, behavioral and physical health needs of clients on a weekly/biweekly/monthly basis.
• Complete in-person nursing assessments and medication/chart review in LTC facilities.
• Educate caregivers and clients on prescribed medications and monitor for side effects.
• Coordinate care with treatment teams, physicians, pharmacies and employment programs.
• Assist clients in building coping skills and annual goal setting related to psychiatric diagnosis.
• Support assigned locations’ clinic and residential needs as required.
• Mentor peers and train colleagues as needed.
• Cover multiple facilities as company needs require.

Responsibilities may include:

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in person nursing assessments as well as medication and chart review in long term care facilities (LTC).
• Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
• Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
• Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May need to cover more than one facility depending on company needs

Requirements for this position include:

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Preferably has experience working in LTC facilities as well as Behavioral Health.
• CPR or Course can be provided during training
