Job summary

ViaQuest Psychiatric & Behavioral Solutions is seeking a Psych RN LTC for full-time work with no weekends to provide comprehensive nursing care in long-term care settings. You will assess, educate, and coordinate care for clients with dementia, anxiety, depression, and other diagnoses, supported by a multi-disciplinary team.

Responsibilities include weekly to monthly health assessments, medication reviews, and care coordination with physicians and staff.

Qualifications

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Preferably has experience working in LTC facilities.
• CPR or course can be provided during training.

Responsibilities

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in person nursing assessments as well as medication and chart review in long term care facilities (LTC).
• Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
• Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
• Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May need to cover more than one facility depending on company needs
