Qualifications

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Preferably has experience working in LTC facilities and Behavioral Health.
• CPR certification or training provided during onboarding.

Responsabilités

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in person nursing assessments and medication/chart review in long term care facilities.
• Educate caregivers and clients on prescribed medications and monitor for possible side effects.
• Care management duties including coordination of community services and communication with treatment team members.
• Assist clients in building coping skills and annual goals related to their psychiatric diagnosis.
• May cover more than one facility or assist clinic/residential needs.

Responsibilities may include:

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in person nursing assessments as well as medication and chart review in long term care facilities (LTC).
• Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
• Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
• Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May need to cover more than one facility depending on company needs

Requirements for this position include:
