Job summary

ViaQuest in Beachwood, OH seeks a registered nurse or licensed practical nurse to provide psychiatric and behavioral health services in long term care settings. The role supports clients with complex diagnoses and collaborates with a team of RNs, LPNs, nurse practitioners, psychiatrists and social workers.

You will perform nursing assessments, educate caregivers and clients on medications, monitor side effects and coordinate care with the treatment team across facilities.

Qualifications

• Graduate of an accredited nursing program.
• RN or LPN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• CPR certification (training provided).

Responsibilities

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in person nursing assessments as well as medication and chart review in long term care facilities (LTC).
• Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
• Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
• Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May need to cover more than one facility depending on company needs

Education

Requirements for this position include:

• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Preferably has experience working in LTC facilities as well as Behavioral Health.
• CPR or Course can be provided during training
