Job summary

ViaQuest Psychiatric & Behavioral Solutions is seeking a nursing professional to support long-term care facilities within the assigned service area. You will perform in-person assessments, med reviews, and caregiver education to optimize patient outcomes.

Qualified candidates hold LPN/RN licensure and have experience in psychiatric/behavioral health, with willingness to travel across facilities. CPR training is provided during onboarding and travel is required as needed.

Qualifications

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Preferably has experience working in LTC facilities as well as Behavioral Health.
• CPR or Course can be provided during training

Responsibilities

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in person nursing assessments as well as medication and chart review in long term care facilities (LTC).
• Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
• Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
• Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May need to cover more than one facility depending on company needs
